SCREENINGS
- Growth, vision, and hearing
screenings are mandated by the PA Department of Health.
- You will receive a referral if
your child does not pass a screening.
- Health care provider to
complete their section and form returned to the nurse.

PHYSICALS
- Mandated by law for entry to school, kindergarten,
6th and 12th grade.
- Any physical one year prior to the first day of the
school year is acceptable.

SLIDE 5
DENTAL SCREENINGS
- Good dental health is the window to good overall health.
- The ESASD Dental Hygiene Services Program’s primary focus is
education and providing mandated dental screenings to students upon original entry, Kindergarten, 3rd and 7th grades.
- In lieu of an “at no cost” school screening, parents may provide a
private dental exam report, signed by your dentist and returned to school by November 1 of the current school year. The report can be dated no earlier than 1 year prior to the opening of the current school year.
- If a private dental report is not provided by a parent/legal
guardian, a school screening will automatically be completed.

IMMUNIZATIONS
- All students in grades K-12 must have immunizations
up to date. Proof of compliance must be turned into nurse’s office by 1st day of school.
- If immunizations are incomplete, the nurse must have
a signed medical plan from a health care provider or student will be EXCLUDED from attending school.

MEDICATIONS
- All medications including over the counter drugs
must have a health care provider’s order and order must be signed by the parent. New orders are required every school year.
- All medications must be brought to the nurse’s office
in the original container.
- Medications must be brought in by the parent NOT
the student.

- All medication needed during the school day require
board approved medication authorization forms.

- Be sure form is completed in entirety, including
parent signature on the bottom.

FOOD ALLERGIES
- Food allergies can be life-
- threatening. If your child
has a life-threatening food allergy, please be sure the nurse is aware.
- Only pre-packaged foods
with nutritional information provided may be brought into school for classroom celebrations.

SCHOOL ILLNESS/INJURY
- When students are seen in the health
room, minor first aid will be provided.
- Parents will be contacted for any illness
- r injury requiring any follow-up.
- It is the recommended that students be
free of fever, diarrhea, or vomiting for 24 hours prior to returning to school without the use of acetaminophen or ibuprofen.
- If uncertain if your child should report to
school, please contact the school nurse.

CHANGE OF CLOTHING
- It is strongly recommended that
all parents of kindergarten students send in seasonal appropriate spare clothing (including shirts, pants, underwear, and socks).
- Accidental spills and wet
clothing from recess happen quite frequently in school.
