Observations on Current Hurdles
- Identifying problems is the first step to improving
- It is natural to see problems and not notice when things work
- “A complaint is a gift”
- Assume good will from others
- Common goals
- Support clinicians who give vaccinations
- Improve user experience
- Improve data quality
- Solutions will require input from all perspectives

Why doesn’t IG support full list of route codes for HL7?
- IG only selected the values that support
Immunization related test cases.
- IGs can constrain codes
- Also done with NDC
- Recommendation
- Clinician should only see limited list
- EHR should only send the limited list to IIS
- If not, EHR should send data anyways
- If the IIS needs this corrected it will signal
in the ACK message with E or W
Sending AR in an Acknowledgement
- Some IIS send AR in MSA-1 to indicate a severe issue or a
problem that prevented any data from being accepted
- Current IG specifies use of AR to very specific scenarios:
- Unsupported Message Type, Event Code, or Processing ID
- Unable to process for reasons unrelated to format or contact
- Acknowledgement Guidance document defines how to
message severe problems:

Sending AR in an Acknowledgement
- ACK Guidance defines how to indicate a severe problem:
- Send AE in MSA-1
- Send E in ERR-4
- IIS who currently use AR off-standard:
- Will need to support ACK Guidance to meet Assessment measures
- Need to communicate these changes to their partners
- EHR systems may need to make adjustments
- Hoping for reduced complexity and confusion by having all IIS use
the same standard
Messaging CDS Evaluations and Recommendations
- IIS should send recommendations for all vaccinations in
standard schedule, including…
- Future vaccinations (HPV)
- Too old to give vaccinations (Rotavirus)
- Receivers are expected to filter for relevant data
- IIS should send evaluation status for all standard
vaccinations administered
- Should indicate which doses are valid
