• Symptom management
• End of life care
• Complex discharge planning
• Complex physical, spiritual and psycho-social needs
• Wide range of incurable conditions including:
• Cancer
• Degenerative Neurological Disorders
• End Stage Chronic illness

Service provides: Face to Face Specialist Clinic Telehealth Specialist Clinic Home Visits Ward Consults Education Clinicians: Clinical Nurse Consultant Palliative Care Physician Counsellor- bereavement support Hospital Liasion Nurse Regions covered: Sunraysia Robinvale Ouyen/Mallee Track Health
