Member Experience

Member Process Flow

1 Initiation Member contacts call center toll free at Davis Vision Line 844.246.0544 to register. (Note: Secure Online Member Registration available 3/1/2017) 2 Registration and Referral EPIC Hearing Counselor registers member; confirms eligibility; and issues referral to local participating provider. 3 Payment Processing After member is seen, provider sends hearing aid recommendation with member’s audiometric results to EPIC. EPIC will contact member to discuss recommendation; benefit coordination (if applicable); and any applicable out-of-pocket expense with payment and financing options. 4 Fitting & Trial Period The hearing aid(s) are ordered by the provider. The member is fit with the hearing aid(s) beginning their 45 day trial period. 5 Finalization At the completion of the 45 day trial period both the member and the provider sign off

• n the hearing aid(s). The member is then mailed a complimentary one year supply of

batteries (per hearing aid) and his/her warranty is extended to 3 years.

*Basic Level products carry a 1 year warranty

Desktop and Mobile Application Members/Employees simply register and complete 4 fun and educational activities to unlock their discount on hearing services and products through their EPIC Hearing benefit. Activities include:

• 1. Watch a video to see what could be missing without healthy hearing
• 2. Take a quiz about hearing loss risks
• 3. Complete an online hearing test to identify signs of hearing loss
• 4. Learn more about stages of hearing loss and hearing gain technology
