Chronic Condition Management (CCM) CPT Code 99490 Collecting and interpreting patient data CPT Code 99091 Reimbursement for non-face-to-face management and care coordination for patients with two or more chronic health conditions Reimbursement for monitoring, interpreting, and intervening (if needed) in response to physiological data from patient devices such as glucometers, blood pressure meters, and weight scales
