for older adults to encourage maximum independence and prosperity.

placement by coordinating home and community- based services for individuals that help them to continue to safely remain at home.

seniors by operating 7 senior centers, 4 adult day care centers, and 15 congregate dining sites with evidence/outcome-based programs.

• ver 500 caregiver families to maintain employment, produce valuable

goods and services, and receive wages to fuel the local economy

• 110,000 visits to our Active Senior Programs and Events in FY19
• More than 2,000 seniors annually enjoy “more than a meal” at our

congregate dining sites

• Provide resources that help seniors stay active and independent
• Via in-home care services, maximize independence for nearly 1,400 at-

risk seniors annually

• Annually provide nutritious meals and counseling to over 2,200

homebound seniors experiencing food insecurity

• Reduce risk of higher health care costs, increased emergency room

visits, fall risks, food insecurities, and isolation

In-Home Care Assistance Helps “Lovebirds” (Ana and Ruben) Remain Together

• Disabled 89 yr-old client with 85 yr-old caregiver spouse
• Multiple health conditions requiring more help than caregiver could

provide

• Consideration of nursing home placement, but not wanting to be

apart

• In-home care services provided in lieu of long-term care placement
• Home together, avoiding emotional distress of being separated

Low-Cost Service Intervention Saved Irene’s Life

• Additional BOCC funding leveraged to assist clients awaiting grant

program services

• Emergency Alert Response service provided to waitlisted applicant

whose screening assessment yielded lower priority ranking

• The senior was able to summon immediate assistance when she

suffered a heart attack

• Immediate attention saved this client’s life, and she was able to

return home, also avoiding long-term care placement
