Donor Alliance Aftercare Program Overview The Aftercare Program strives to provide comfort, support, and education to donor families by offering the following : • Recipient and donor family correspondence • Quilt program • Holiday card • One year anniversary card • Annual Donor Family Tribute • Online bereavement newsletter • Grief resources and literature • Memory cards

Memory Card Overview • Offered to all families of donors • Provided at no cost to the family • Standard order is 100, but can do more • Can still be provided for donors who are ruled out for donation after medical screening is completed – E.g. positive serological tests

Basic Elements • Photo • “Giver of Life Through Donation” • Words of Tribute (poem, verse, quotation, etc.) • Information on how join the state donor registry in memory of the donor

When Families C an’t Find a Photo • Check Facebook or LinkedIn profiles • Online obituary (image scanned and uploaded by funeral home) • Internet Search – workplace website • Use an image of a nature scene or something meaningful to the donor or family
