program and the quality of services that families are receiving… It does not mean perfection!

regarding your target population?

  • Have you clearly identified your community

partners? Would you be rated a 2 or 3?

  • Are you following your local screening process

listed in your PPPM?

  • Are at least 80% of your screens occurring either

prenatally or within the first two weeks after the birth of the baby?

  • Are at least 80% of first home visits occurring

prenatally or within the first three months after the birth of the baby? Initiate services prenatally or at birth.

time after a first home visit when disengaging from services, conducting activities during the course of creative outreach and continuing creative outreach for at least three months? Offer services voluntarily and use positive, persistent

family trust.
