  • The data analyze reflect the audit period: March 1st,

2016 to February 28th, 2017

  • Baseline Data: 42 Individuals, 39 Not Englewood, 3

Englewood

  • Individuals with a developed or updated treatment plan = 19

(45%), Not Englewood = 18 (46%), Englewood = 1 (33%)

  • 10 Individuals were Phone Screened and Refer Out

  • Found documentation issues on the team

 Create documentation expectations, what should be included and what shouldn’t. Define the standards on how information should be documented.

  • Found that some charts without treatment plan were

clients assigned to group therapy.

 Take a look into this and decide how could be solved.

  • Sustained Improvement: Continue completing chart

audits to assure that proper documentation is complete and to identify issues that need to be resolve.
