Background ■ Capital & Coast and Hutt Valley DHBs had a combined P&F team ■ But this divided at the beginning of the year ■ However combined P&F didn’t stop people being stuck in hospital or elsewhere “between NASCs” ■ Silos within provider arm ■ Perceived silo’s between NASCs

Formal process established ■ Governance group established to sort these issues and resolve ‘stuck’ cases ■ Regular monthly meetings – clinical chair independent from NASC – administration independent from NASC ■ Clear expectations from all NASCs to clinical DHB teams re referral of ‘stuck’ cases ■ Formalised communication between NASCs

Early learnings ■ Importance of – Relationships within wider DHB – Relevant information provided with referral – Collective NASC position – Relationships between NASCs ■ Identification of systemic issues e.g. accommodation ■ Challenges of rigid allocation criteria
