Social service/wraparound support

  • CBO staff provide:
  • Wraparound: Meet short term immediate needs for

isolation & quarantine (e.g., food, motel/hotel, etc.)

  • Safety net: Connect to established social service

programs for long term needs (e.g., SNAP, housing, OHP)

  • Screening and assessment to determine need
  • Ask if person already connected to existing supports:
  • e.g. CCO member or DHS disability programs
  • Refer to existing supports
  • Regional Outreach Coordinator/OHP enrollment assister
  • DHS liaison

Role of CCOs

  • CCOs retains medical case management responsibilities

for members who are in isolation or quarantine due to COVID-19.

  • LPHA receives reports of positive cases. LPHA/CBO

social support case management is an additional benefit and resource for those members.
